Good morning. It's September 13.
When Maria Branyas died at 117, she was the world's oldest verified person. Scientists just published the most detailed biological analysis ever done of someone who lived that long. What they found was not what anyone expected.
The rundown this week:
Semaglutide linked to nearly 40% fewer asthma attacks in a large real-world study - another anti-inflammatory effect nobody was looking for
A new DNA-based treatment just cut bad cholesterol by nearly 50% without statins
Cancer now affects 1 in 5 people in their lifetime globally - what that means for longevity medicine
MIT engineered injectable mini livers that work inside the body
Let's get to it. 👇

Eating only between noon and 8pm improved blood sugar control in adults with Type 1 diabetes and obesity - a small but promising study found that time-restricted eating reduced glucose levels without increasing the risk of serious complications. ScienceDaily
Nearly 21 million people were diagnosed with cancer globally in 2024, and about 1 in 5 people will develop it during their lifetime - survival rates vary enormously by geography. The data confirms cancer is now the defining chronic disease of the longevity era. WHO / IARC
An experimental compound called AP503 significantly strengthened bones in mice by activating GPR133, a receptor that boosts bone formation while slowing bone loss - osteoporosis affects half of women and a quarter of men over 50. A new mechanism for targeting it is long overdue. ScienceDaily
People who reach 110 and beyond appear to have unusually large numbers of rare immune cells capable of killing abnormal cells - their CD4 cytotoxic T cells became more common with age rather than declining. The immune system of a supercentenarian doesn't just survive - it adapts. EurekAlert
MIT engineers developed injectable "mini livers" that work inside the body - tiny clusters of liver cells encapsulated in a way that survives inside the body and performs liver functions. A proof of concept for organ support without transplant. Nature Medicine / MIT

The Biology of the World's Oldest Person - And What Actually Got Her There

By Arxiu de la familia Branyas Morera - Public Domain, Wikimedia Commons
Scientists at the Josep Carreras Leukaemia Research Institute in Barcelona analyzed blood, saliva, urine, and gut microbiota samples taken from Maria Branyas when she was 116. The goal: understand what made her different from every other person who had ever lived that long and stayed that healthy.
What they found on the aging side was expected. Very short telomeres - the protective caps on chromosomes that shorten with every cell division. An aged B lymphocyte population. A pro-inflammatory immune profile. Her cells bore every mark you'd expect from 116 years of living.
What they found on the protection side was not.
Epigenetic age 10-15 years younger than chronological - at 116, her biology functioned closer to someone just over 100. Her cellular age and her calendar age had separated significantly.
Gut microbiome dominated by Bifidobacterium - the gut bacteria found in yogurt and fermented foods, associated with immune protection and reduced systemic inflammation. She ate yogurt regularly throughout her life.
Low actual inflammation - despite the pro-inflammatory immune markers, real-world inflammation was suppressed. This reduced her lifetime risk of cancer and diabetes.
Efficient fat and cholesterol metabolism - maintained throughout her life, protecting her cardiovascular system.
Genetic protection for heart and brain - favorable variants that defended against degenerative disease.
Short telomeres actually protected her from cancer - by limiting uncontrolled cell multiplication, a mechanism that reversed their usual role as a negative aging marker.
And her lifestyle? Researchers were clear it played a central role alongside the genetics.
Never smoked. Never drank alcohol. Not once, across 116 years.
Maintained a healthy weight throughout her life.
Balanced Mediterranean-style diet - with regular yogurt consumption directly feeding that bifidobacteria-rich microbiome.
Strong social connections - diversified, with both friends and family - maintained across her entire life.
Lead researcher Manel Esteller: "For the first time, we were able to separate old age from disease."
That's the line. Her body aged. Her cells carried all the expected damage of extreme old age. But she did not get the diseases that aging usually brings. The protection package she ran alongside the aging - partly genetic, partly behavioral - kept disease at bay for 117 years.

Gravity Water Filters: Berkey vs Boroux vs Rorra
Countertop gravity filters are the longevity crowd's answer to plastic water bottles and unfiltered tap water. No electricity, no plumbing changes, no ongoing plastic waste. Three brands dominate the conversation: Berkey, Boroux, and Rorra. Here's the breakdown:
💧 Berkey
Price: $367 (Big Berkey 2.25 gal with 2 Black Berkey elements); fluoride filters add ~$50 extra
Filtration mechanism: Dual-stage gravity - carbon block elements remove pathogens, chemicals, and heavy metals; optional PF-2 fluoride filters add a second stage
What it filters: 200+ contaminants - bacteria, viruses, lead, mercury, arsenic, PFAS, pharmaceuticals, pesticides, chlorine, VOCs. Fluoride only with the add-on PF-2 filters (100% removal in testing)
Filter lifespan: 6,000 gallons per pair (~2-3 years for a family of 4). Very low ongoing cost at ~2-3 cents per gallon
Machine lifespan: Stainless steel - essentially indefinite if maintained
Caveats: Lab testing found leaching of 1.44 PPM aluminum - nearly 3x the recommended limit. The 6-size range and fluoride add-on capability are genuinely useful
Plastic-free: Mostly stainless steel, but the spigot contains plastic. The Berkey Light model is BPA-free plastic throughout - avoid it
Certificates: No NSF/ANSI certification. The brand had an EPA dispute over marketing claims.
💧 Rorra
Price: $549 (2.5 gal)
Filtration mechanism: Single-stage gravity filtration. Includes smart sensors that tell you how much water is left and when to replace the filter
What it filters: 50+ contaminants - uranium, barium, copper, zinc (all 100% removal in testing), heavy metals, PFAS. Removes 0% fluoride
Filter lifespan: 90 days or 200 gallons per filter - mandatory subscription required. Ongoing cost is significantly higher than Berkey or Boroux
Machine lifespan: Newer brand, well-built - but less track record than the others
Caveats: The mandatory subscription and narrow contaminant range (50+ vs 200+) are the main trade-offs for the smart features and premium design
Plastic-free: Nearly plastic-free design - the best in this category aesthetically and materially. 25-minute setup, no filter priming required
Certifications: No NSF, WQA, or IAPMO certification. Limited independent lab data published.
💧 Boroux
Price: $355-455 (Legacy 3 gal; available in silver, black, or white)
Filtration mechanism: Same gravity-fed carbon block mechanism as Berkey. Foundation filters are compatible with Berkey housings and are built to the same spec - but published with full third-party data
What it filters: PFAS, heavy metals, chlorine (95%+), microplastics, particulates - similar breadth to Berkey. No fluoride removal
Filter lifespan: Foundation filters rated comparable to or longer than Black Berkey elements. Similar per-gallon cost
Machine lifespan: 100% stainless steel - indefinite
Caveats: Few size options. Just like Berkey it has no fluoride removal.
Plastic-free: The best of the three in this category - fully stainless steel, no plastic versions, component quality rated 10/10. Built by former Berkey distributors who fixed the EPA and certification issues
Certifications: NSF/ANSI/CAN 372 (lead-free materials) + NSF for microplastic and particulate filtration. Publishes full third-party spec sheets - more transparent than Berkey
Bottom line: Berkey is the widest contaminant coverage including fluoride. Rorra is the most modern and plastic-free but costs significantly more to run long-term and removes fewer contaminants. None of them remove fluoride except Berkey with the add-on filters. Boroux is the most well-rounded - stainless, certified, transparent test data, a good price, and the Berkey formula without the EPA baggage. Check out the Boroux website here.

A DNA Treatment Just Cut Bad Cholesterol by Nearly 50% Without Statins
Researchers designed a new DNA-based treatment targeting PCSK9 - a protein that reduces the liver's ability to clear LDL cholesterol from the blood. Block PCSK9, and LDL drops sharply. The result in this study: nearly 50% reduction.
PCSK9 inhibitors already exist as injectable drugs - Repatha, Praluent - but require ongoing injections and cost thousands per year. A DNA-based approach could be more durable, potentially requiring fewer doses or even a single treatment.
Why it matters: high LDL is one of the most significant modifiable cardiovascular risk factors. Statin intolerance affects roughly 10% of patients who need cholesterol management. A DNA-targeting alternative that works as well without the side effects would be clinically significant - and cardiovascular disease is still the top cause of death globally.
Early days. But the mechanism is clean and the results are striking.

GLP-1 Drugs Keep Finding New Conditions to Treat. Here's the Market Reading. 📊
A real-world study this week linked semaglutide to 40% fewer serious asthma attacks. This keeps happening - heart disease, kidney disease, sleep apnea, now asthma - because GLP-1 receptors are distributed throughout the body. When systemic inflammation drops, multiple organ systems benefit simultaneously. The indication list is expanding faster than the drug labels can keep up.
The market has been pricing this in for two years. Nearly 50 million US adults have now used an injectable GLP-1, making "eating like you're on Ozempic" the second-most-common diet in America, per Fitt Insider. JPMorgan estimates the behavioral shifts GLP-1 adoption drives could cost the food industry $30-55 billion by 2030 - not from the drugs, but from 50 million people eating differently.
The fitness industry faces a specific paradox: GLP-1 users spend 4x more on wellness than the average consumer, but physical activity tends to drop after starting medication. That gap - high spend, lower movement - is a large structural opportunity for whoever builds the right programming around it.
WADA is now debating whether to ban GLP-1 use in elite sport, after sports medicine physicians estimated that 12% of athletes are using them without a medical need.
The drug was approved for diabetes. It is quietly reclassifying as general-purpose anti-inflammatory infrastructure for the human body.

Ikigai: The Japanese Secret to a Long and Happy Life
Héctor García and Francesc Miralles
The most important thing scientists found in Maria Branyas wasn't a supplement or a biomarker. It was that she maintained strong social bonds for 116 years. This book is about why that matters - and why Okinawa, where centenarians are common, revolves around a concept called ikigai: your reason to get up in the morning. Not what you eat. Why you bother. Short, readable, and quietly makes the case for the most underrated longevity variable we know of.
